Transportation Waiver
TRANSPORTATION WAIVER AND RELEASE
I, the undersigned HEREBY GIVE MY PERMISSION for any representative of the Truro Centurions Swim Club (TCSC), including without limitation, parents or coaches, to transport the named Swimmer to and from any activities, meets, events or other team functions (Events) that they attend, and I will assume all liability for their participation in said Events and any injury that may result during the transport or at such Events.
Further, by signing below I agree as follows:
1. I will not hold TCSC, its officers, agents, employees, assigns or anyone acting on its behalf, responsible or liable for injury occurring to the named Swimmer in the course of such Events or travel;
2. I hereby accept financial responsibility for personal items lost by the person identified herein;
3. I authorize TCSC to transport and to obtain any emergency medical care that may become reasonably necessary for the named Swimmer in the course of such Events or such travel, and agree to accept any associated costs of travel or treatment;
4. I accept full responsibility and hereby grant permission for my minor child (Swimmer) to travel with TCSC.
TRANSPORTATION PERMISSION
I hereby authorize the identified Swimmer to be transported by any TCSC COACH or PARENT in their PERSONAL VEHICLE or RENTED VEHICLE, or to be accompanied by any TCSC COACH or PARENT on any BUS or other MODE of MASS TRANSIT TO AND/OR FROM ANY EVENT.
TRANSPORTATION SAFETY
I, the undersigned, understand, that all persons transported by TCSC must adhere to safety rules, including, without limitation, provincial legislation. Persons must remain seated, wear a seatbelt and follow the driver's directions at all times.
THIS IS A RELEASE OF LIABILITY AND WAIVER. I HAVE READ THIS ASSUMPTION OF RISK, RELEASE OF LIABILITY, HOLD HARMLESS AND INDEMNIFICATION AGREEMENT AND CONSENT. I UNDERSTAND THAT I AM RELEASING RIGHT BY SIGNING IT. I AM EXECUTING THIS ASSUMPTION OF RISK, RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT VOLUNTARILY.
